Care Logistics Care Progression Components

As hospitals focus on Care Progression as a core initiative to provide financial gains and services expansion, Care Logistics is now offering our members the option to deploy Care Progression components of their Hospital Operating System™

Formerly offered as a package, Vizient MidSouth members can now purchase Care Logistics’ Hospital Operating System™ components separately and later phase in the adoption of the overall system. This will enable members to spread out the cost over a longer period of time.

The Care Progression component offerings are focused on achieving dramatic improvement in quality patient progression (QPP) measures while supporting regulatory and compliance requirements. Here is a snapshot of technology enabled services components that can be individually implemented:

Care Progression Optimized – Patient statusing, patient progression plans, nursing/ED throughput
  • QPP Measures: LOS reductions, CMI increases, capacity throughput and growth
  • Compliance: Mandatory bundles, 2-midnight rule, CMS short stay reviews, SNF 3-day stay rule, CMS D/C proposed rule
Operational Transformation – Engineering and nursing services to implement hub and spoke care coordination utilizing existing technology infrastructure
  • QPP Measures: Significant LOS reductions, CMI increases, LWOT reductions, capacity throughput and growth, stronger HCAHPS scoring, patient safety measures
  • Compliance: Mandatory bundles, 2-midnight rule, CMS short stay reviews, SNF 3-day stay rule, CDI compliance and audit, readmission reduction program, Joint Commission EP standards
Order Logistics – Managing simultaneous order volume and service area throughput
  • QPP Measures: Reducing diagnostic operating costs, increasing outpatient growth, LOS reductions
  • Compliance: HCAHPS, HACRP, Mandatory Bundles
Intake Logistics – Patient admission to appropriate care level/setting
  • QPP Measures: Capacity throughput and growth, admission efficiency, identify readmission risks
  • Compliance: EMTALA

The deployment time for most of the component packages is less than nine months, with a documented return on investment of 5x to 10x. Care Logistics also utilizes a methodology that creates financial returns during the deployment. Hospitals can achieve this significant return on investment utilizing their operating expense budget with a much smaller capital outlay.
